Founded in 1978, we are a not-for-profit provider of skilled, compassionate, and culturally sensitive hospice, palliative care, and home health agency services. We serve patients and families throughout Palm Beach and Broward counties, delivering 90 percent of our care and support in patients' homes (private residences, assisted living facilities, and skilled nursing homes), and the balance in hospitals and our inpatient care centers in Boca Raton and Hollywood.
