Greystone Health Network: Passionate People Providing Compassionate Care

Healthcare Management – Comprehensive operational and clinical management, including maximizing profitability, survey compliance, improving customer service and satisfaction, and achieving optimal outcomes.
 
Health Centers – Rehabilitation services, including complex skilled nursing management, in-house dialysis, nutrition management, complete IV management, complex wound care, tracheostomy care, specialized rehabilitation programming and AlterG® Antigravity Treadmill Rehabilitation. The Greystone Health Network includes the only post-acute provider in South Florida offering AlterG.
 
Home Health Services – Four home health agencies designed to extend skilled nursing services and offer a seamless transition from inpatient care to the home.
 
Greystone pioneered a homecare program called “Good Night SleepTight,” which ensures that every client receives a call to check on them before they retire for the evening, and “Moments of Mending,” which provides a check-in phone call within 72 hours of admission making sure clients are eating well, resting comfortably, and that they have everything they need.
 
Outpatient Rehabilitation – Physicial, occupational and speech therapy, with transportation provided to patients as needed.

• A Transitional Care Program where a nurse navigator guides patients within the Greystone Health Network to the right place at the right time. The nurse oversees the patient’s entire plan of care, making the transition seamless, and also helping to engage the patient with community resources and medication/dietary compliance.
 
• Centers of Excellence in its facilities for cardiac care. The Centers of Excellence are a part of a company-based program including advanced protocols and extra CEU training specific to cardiac patient care. This specialized program includes full-time advanced practices nurses.
 
According to Vasquez, these plans help set Greystone ahead of the game in managing today’s industry challenges as well as withstand the impact of tomorrow’s changes.
 
“We are seeing a migration of sicker patients coming into our sector,” he said. “With a concurrent compression of reimbursement, we must do more with less and heighten our competency to take care of patients while optimizing every dollar.”
 
The Greystone Health Network is adapting an electronic medical record (EMR) to achieve a paperless environment. “The goal is to interface with hospitals and managed care partners so medical records will follow patients at the push of a button,” he said. “Ultimately, all information will go back to the primary care physician so the doctor has a chance to review and make clinical decisions based on the patient’s journey.”
 
“That’s the overall concept behind the Affordable Care Act – having primary care physicians being the spoke of the wheel, so they can provide the best course of care and prevent rehospitalization,” he added.
