High Blood Pressure (Hypertension)

High blood pressure, also known as hypertension, is a common disease that develops when blood flows through your arteries at higher-than-normal pressures. Your blood pressure is made up of two numbers: systolic and diastolic. Systolic pressure is the pressure when the ventricles pump blood out of the heart. Diastolic pressure is the pressure between heartbeats when the heart is filling with blood. 

Diabetes

Diabetes is a disease that occurs when your blood glucose, also called blood sugar, is too high. Blood glucose is your main source of energy and comes from the food you eat. Insulin, a hormone made by the pancreas, helps glucose from food get into your cells to be used for energy. Sometimes your body doesn’t make enough—or any—insulin or doesn’t use insulin well. Glucose then stays in your blood and doesn’t reach your cells.

Heart Failure

Heart failure is a condition that develops when your heart doesn’t pump enough blood for your body’s needs. This can happen if your heart can’t fill up with enough blood. It can also happen when your heart is too weak to pump properly. The term “heart failure” does not mean that your heart has stopped. However, heart failure is a serious condition that needs medical care.

COPD
Chronic Obstructive Pulmonary Disease and Emphysema

COPD, or chronic obstructive pulmonary disease, is a progressive disease that makes it hard to breathe. Progressive means the disease gets worse over time.

Cancer

Cancer patients receiving chemotherapy must not be running a fever or have other symptoms of illness when receiving their treatment. RPM allows these patients to check their vitals before making the trip to the treatment facility.
