I am a senior consultant with Healthcare Navigation Systems; a fairly new company that provides consultation and support for outcomes improvement initiatives. Our team is experienced in the health care delivery system and focused on contributing value to healthcare providers.

Our experience includes, but is not limited to:
• Providing comprehensive support in piloting Medicare Bundle Payment Care Improvement (BPCI) initiative with NYU Langone Medical Center. The BPCI initiative is sponsored by the Center of Medicare and Medicaid (CMS) to improve patient outcomes and reduce cost. The program was launched by the hospital in 2011.
• Development and implementation of key strategies to reduce length of stay (LOS) and readmissions rates.
• Developing patient centered LOS reduction initiatives to improve outcomes, and reduce cost.
• Providing exceptional care coordination for patients over a 90-day episode of care and ensuring full patient support.
• Effectively managing all aspects of the patient care transition process, coordinating care for patients across the care continuum.
• Assisting in the development of standardized care pathways to reduce LOS and managed post discharge follow up support.  
• Supporting the development of Epic (EHR) dashboards and registry to assist with effective care coordination.
• Assisting in developing policies and procedure for workflow focused on care coordination; improving on outcomes and reducing cost.
• Experience in managing gaps in care.
• Use of data analysis to identify and test the most effective means for engaging different types of patients.
• Effective use of HIE to improve communication among stakeholders; hospitals PCPs, NPs, and specialists to improve care coordination.
• Use of PHI provided by PCPs; to categorize patients based on acuity; to inform the focus of care coordination.
