For Tra’Keisha Tatiana Patrick, MSW, RSWCI, case management is about seeing the whole person and creating a path forward during life’s most uncertain moments. After six years in skilled nursing and long-term care, Patrick joined Memorial Healthcare System seven years ago, growing from social worker to leader while remaining grounded in advocacy, education and compassion.

As social work supervisor of case management, Patrick provides daily leadership to social workers and nurse case managers, navigating complex patient needs and barriers to discharge while connecting families with needed resources. Even as a leader, she values opportunities to step back onto the front lines, supporting patients and families while staying closely connected to her team’s work.

Some of her most meaningful experiences involve guiding families through the transition to hospice care. “End-of-life care is not simply a countdown to death; it is an opportunity to focus on comfort, dignity and quality of life,” Patrick said.
