Taylor Adams began her Memorial Healthcare System career as a bedside nurse on a neuro telemetry unit but was intrigued by a fellowship program that offered an easy transition to case management. “It provided an opportunity to explore another type of nursing, while still focusing on patient care. Coming from a clinical background, I’m able to anticipate how long their stay may be and what their needs will likely be at discharge.”

That discharge plan – whether to home, rehabilitation or skilled nursing facility – begins with listening to patients and understanding their goals. Adams says being the patient’s advocate is an important part of the case manager’s role, and the opportunity to communicate with families and staff at Memorial Regional what she enjoys most about the position. “It’s critical that everyone be on the same page to ensure a smooth transition for those we’ve cared for.”
