FoundCare, Inc., is proud to announce it is a recipient of the Special Project of National Significance Grant. The funding, which was possible through the Health Resources & Services Administration, is to promote the design, implementation, and evaluation of system-level change in staffing structures that improve health outcomes along the HIV Care Continuum.

 

FoundCare will receive $1.2 million over four years and was one of only 15 program grantees in the United States. “This award is exciting as the epidemic of HIV/AIDS is changing, we need to design a more efficient way to deliver comprehensive, quality healthcare to those living with HIV/AIDS in Palm Beach County. Now, with this funding we can begin to integrate these patients in a Patient-Centered Medical Home Model by creating seamless linkage to affordable care all under one roof,” said Zack Sharp, FoundCare’s Director of Health Services.

 

FoundCare, Inc. has a history of creating innovative and diverse solutions to address the healthcare needs of Palm Beach County residents. FoundCare’s Comprehensive AIDS Program (CAP) is the oldest and largest program serving people with HIV/AIDS in Palm Beach County since 1985. Realizing the need for additional healthcare services for the medically underserved population (without HIV/AIDS) FoundCare created FoundCare Health Center in 2009 and became a Federally Qualified Health Center (FQHC) in 2013.
