December 16, 2025 – Healthcare Network is pleased to announce it has received a $40,000 grant from the Collier Community Foundation, expanding the organization’s capacity to provide enhanced telehealth services for 500 high-risk pregnant and postpartum patients, as well as uninsured or underinsured individuals living with chronic conditions.
These funds will support the Remote Patient Monitoring (RPM) Program, a form of telehealth that improves quality of care for chronic disease treatment and reduces hospital readmission rates, with the purchase of 250 sets of Bluetooth-enabled RPM devices.
Tools such as blood pressure cuffs, scales, glucometer kits, and secure gateway (monitoring) devices will help capture and transmit real-time patient data directly to Healthcare Network, allowing medical teams to respond swiftly to concerning health trends. To reduce any barriers to use, care coordinators work directly with patients on how to use the devices, which feature simple interfaces and gentle visual reminders designed to increase adherence and support long-term health.
“Many of our patients encounter significant barriers to care that make it difficult to receive in-person care. This is very concerning for high-risk expectant mothers and individuals with chronic conditions who require more frequent monitoring and follow-up appointments,” said Jamie Ulmer, CEO of Healthcare Network. “This generous support from the Collier Community Foundation allows us to reach these vulnerable patients with technology that keeps them safer, healthier and more connected to their care team, ultimately leading to better, life-changing health outcomes.”
The Collier Community Foundation awards annual grants through a competitive application and review process. Selected organizations receive grant dollars from the Foundation’s discretionary funds. These funds were gifted by donors who believe in the Community Foundation’s ability to mobilize resources to address the community’s greatest needs.
