This innovative solution enables continuous at-home monitoring of critical health metrics, providing real-time insights into patient health status and significantly improving chronic disease outcomes.
HealthSnap empowers the medical center’s physicians to closely monitor key health indicators, including blood pressure, blood glucose, weight, and oxygen saturation. This proactive approach allows clinicians to quickly respond to concerning patient values by adjusting medications, dietary recommendations, or arranging timely follow-up care, effectively reducing unnecessary emergency room visits and hospital admissions.
The main application involves monitoring blood pressure, an essential factor in managing chronic health conditions. Our data has shown clinically meaningful reductions in both systolic and diastolic blood pressure among participating patients. Achieving better blood pressure control and improving medication adherence substantially lowers the risk of serious complications such as cardiovascular and kidney disease.
The RPM initiative is rapidly scaling across 13 Mount Sinai locations throughout South Florida, poised to serve up to 10,000 patients. This comprehensive program delivers proactive management for a range of chronic conditions, including hypertension, diabetes, congestive heart failure, COPD, asthma, obesity, and osteoporosis.
