For Claudia Wenger, RN, case management is about seeing the whole picture and helping patients and families find their way forward. A nurse since 1990, Wenger spent years at the bedside, gaining experience throughout the hospital on medical-surgical floors, the ICU and emergency room, before transitioning to case management 20 years ago.

That clinical experience helps Wenger anticipate what patients may need next. She reviews medical records, assesses patients and works with physicians, families and care teams to determine whether a patient may need rehabilitation, home services, medications or other support after discharge.

Her work also extends beyond medical needs. Wenger helps patients experiencing homelessness identify housing options, assists those without insurance and connects patients with community clinics and other resources.

“I love being able to guide patients and families in a direction where they can get the help they need, whether they are in the hospital or when they leave,” she said.
