A nurse for more than 25 years, Donnette Grimes is now helping patients reach their goals as a chronic case manager on the care team. She says building a trusting relationship is the key to assisting those who may lack knowledge and fear the unknown when it comes to their own health. “We have many different nationalities and cultures we work with, so it’s important I meet them at their level and speak their language. Some have traditionally not trusted or even believed in medicine, so our monthly outreach helps break down barriers.”

Grimes makes referrals to specialists, assists with prescription refills, appointments for labs, and other technical challenges that may arise. She provides patients her direct phone number and wants them to know she will always be an advocate for their health.

When asked how she views all she’s accomplished during a lengthy career, Grimes replied, “success is liking yourself, what you do, and how you do it.”
