Laura Agudelo has found her calling in case management, helping patients and families navigate complex transitions with confidence and peace of mind.
Working primarily with inpatient rehabilitation for adults receiving care after a stroke, orthopedic surgery, cardiac event, or neurological condition, Laura coordinates discharge plans, insurance requirements, medical equipment and follow-up services in close collaboration with interdisciplinary care teams.
She feels rewarded when a complicated discharge plan comes together and a patient can safely move to the next stage of recovery with the right resources in place. Balancing clinical needs, insurance coverage, family expectations, and available services requires organization, flexibility and strong communication.
Her advice to those entering the field reflects what she has learned firsthand: “Stay organized, communicate well, ask questions and always be ready to adapt.”
