For several years, Gloria Hasbun has helped guide patients and families through important transitions in rehabilitation care. As Director of Case Management at St. Catherine’s West Rehabilitation Hospital, Gloria coordinates care from admission through discharge, collaborating with patients, families, physicians, nurses, therapists and insurers.
She primarily supports patients recovering from stroke and orthopedic conditions who require intensive rehabilitation before returning home or transitioning to the next level of care.
For her, the most rewarding moments are seeing patients regain independence and leave the hospital with the support they need. Her approach to successful discharge planning requires organization, communication and careful coordination of insurance, equipment, transportation and follow-up services.
Her advice to new case managers: “Be patient, stay organized, listen, and remember that every patient has a story. Above all, treat people with compassion and respect.”
