
No two days in case management are the same for Erika Solis. From aging and homelessness to substance use and complex family dynamics, she understands that each patient brings unique circumstances that require critical thinking, compassion and often a little creativity. Solis first discovered her passion for hospital social work while interning at Memorial during her master’s program.
As social work manager at Memorial Hospital Pembroke, Solis helps patients navigate unfamiliar resources and discharge planning while supporting her team in breaking down complex cases and finding a path forward. “For patients trying to get through the day with no support or assistance at home, it’s like we become their family member while they are admitted,” Solis said.
She recalls one patient whose prolonged hospital stay could have ended very differently. By exploring every option, the team successfully helped the patient return home rather than move into a nursing facility.
