
Helping families feel supported during some of their most difficult moments is at the heart of the work of Tawnya Stack, RN. As a pediatric intensive care unit nurse case manager at Joe DiMaggio Children’s Hospital, Stack coordinates care across medical teams, families and community resources while navigating discharge planning and barriers to care.
One complex discharge reinforced for Stack just how meaningful that coordination can be. By bringing together the medical team, family and community resources, she helped address barriers that could have delayed a child’s transition home. The family ultimately left feeling prepared and confident about what came next.
“I’m motivated by building relationships, solving complex challenges and knowing that my support can help families feel less overwhelmed and more confident during difficult moments,” Stack said.
For her, case management is an opportunity to make a difference beyond clinical care, ensuring families feel informed, supported and prepared throughout their child’s journey.
