October 11, 2022 – The Florida Department of Health in Miami-Dade County is pleased to announce it will be hosting virtually the 2022 Annual Community Health Improvement Plan (CHIP) Review Meeting to review the status of the Community Health Improvement Plan (CHIP) and provide updates on health equity in Miami-Dade. This virtual event will be hosted on Friday, October 28, 2022 from 10:00 a.m. to 11:30 a.m. via Microsoft Teams.

The Florida Department of Health in Miami-Dade County uses the Mobilizing for Action through Planning and Partnerships (MAPP) process which is a national evidence-based best practice from the National Association of County and City Health Officials (NACCHO). MAPP is a community-driven strategic planning process used for improving community health by identifying strategic issues from community assessments and community concerns. This process helps set priorities and implement evidence-based initiatives to advance health.

During this meeting, updates will be shared on the new MAPP 2.0 framework that will be released by NACCHO in 2023 which will guide our community health plan process. This is a dynamic process that can improve the efficiency, effectiveness, and ultimately the performance of public health systems. The MAPP Process has also helped us to identify higher rates of chronic disease and where existing health disparities and inequities exist in Miami-Dade County.

The Florida Department of Health in Miami-Dade County launched the Health Equity Office in June 2021 and the Health Equity Office Advisory Committee in December 2021. Our goal is to reduce disparities and improve health equity, especially in vulnerable and disadvantaged populations. Updates will be shared by the Health Equity Office on the progress of health equity projects and their impact on the social determinants of health that influences Miami-Dade County’s community health.

This community meeting will provide community members, leaders, and organizations the opportunity to build on the progress we have made together collaboratively and inclusively on addressing health disparities. The Florida Department of Health in Miami-Dade County seeks to unite decision makers across multiple disciplines to continue this health equity sustainability conversation to develop further strategic approaches to ensure equity for all Miami-Dade residents. Attendees will have the exciting opportunity to learn more about resident health, trends in health outcomes, and current initiatives.
