Action Network (MD-HAN) in response to the increasing demand on the local fragmented healthcare safety-net and the high cost of uncompensated care. In 2012, realizing the importance and potential of the Network, HCSF launched the MD-HAN’s Health Access Initiative (HAI). The HAI’s main focusareas during the next two years are to effect positive systemic changes that increase collaboration and enhance the capacity of Miami-Dade’s safety-net to address the needs of the underserved and uninsured.
 
MD-HAN’s HAl Structure
The MD-HAN is a four tiered structure, composed of existing Community Partners, a Steering Committee, four Work Groups and the new Transformational Leadership Council (TLC).
 
Transformational Leadership Council (TLC)
A select group of policy champions and influential community leaders charged with supporting the initiative’s system change efforts to make them truly sustainable.
 
Steering Committee
Actively promotes and prioritizes goals and objectives ensuring achievement of the HAI outcomes. It monitors, reviews, evaluates and aligns ongoing program implementation with supportive policy. Its four Work Groups are platforms for knowledge exchange, best practice adaptation and planning.
 
Work Groups Community Health Worker/Patient Navigator (CHW/PN)
Exchanges and disseminates best practices/lessons learned throughout the Network contributing to the enhancement of service delivery among CHWs and PNs.
 
Primary Care
Examines access issues and collaborates towards finding solutions focused on improving primary care and the local safety-net.
 
Technology
Provides expert analysis and recommendations on technology solutions in support of a coordinated healthcare delivery system.
 
Sustainability
Develops and refines sound direction to guide the Network, as market and stakeholder expectations change, which supports and advances the Mission and Vision resulting in significant value over time.
