January 20 2022 – Holy Cross Health is committed to helping the aging population in South Florida live their healthiest and most active lives. Most recently the non-profit hospital opened the Holy Cross AgeWell Center at the Holy Cross HealthPlex, 1000 NE 56th St. in Fort Lauderdale, which is a comprehensive, collaborative center focused on healthcare, well-being and social interaction for those who are Medicare eligible.
The AgeWell Center is particularly focused on the care of patients who are enrolled in Medicare Advantage plans. Holy Cross Health now participates in several Medicare Advantage networks, including but not limited to Oscar, AvMed and Florida Blue.
Holy Cross Health is excited to announce a new partnership with Humana and the AgeWell Center to include its Humana Medicare Advantage HMO members. Holy Cross Health already participates in the Humana Medicare Advantage PPO network.
“With this partnership, even more participants will have access to the highest quality care across the Holy Cross Health system as well as access to our specialists, providers and services,” said Holy Cross Health President and CEO Mark Doyle. “This is particularly important to the Medicare population.”
Among the clinical services offered at the AgeWell Center are primary care and multispecialty physicians; pharmacist consults; group clinical visits; nutrition/dietary counseling; occupational therapy; an on-site lab; assistance filling prescriptions during visits; on-site imaging including X-ray, ultrasound, CT, bone density and MRI; physical therapy; and wound care.
In addition to healthcare and clinical services, AgeWell Center members have access to the Blessed Bistro; cooking demonstrations and healthy cooking classes led by Holy Cross chefs; exercise programs with certified fitness instructors; financial and benefits counseling; educational health and lifestyle sessions; spa services; spiritual care with multi-denominational chaplains; support groups to assist with life challenges; and access to community education rooms.
Presently, any patient on Medicare or enrolled in a Medicare Advantage plan, with a primary care provider in the Holy Cross Medical Group, can use the services offered at the AgeWell center.
