Holy Cross Physicians Partners has named Crystal Lyerla, R.N., to the new position of population care manager.Lyerla brings nearly 20 years of healthcare experience to her new position. She has experience as a health coach with Health Dialog, a leading provider of healthcare analytics and decision support headquartered in Boston. She also has extensive experience with direct patient care in hospital, nursing home and home health settings.
Lyerla is a registered nurse, licensed practical nurse, certified nursing assistant, and graduated from Glendale Community College with an associate’s degree.
