On Friday, June 14, from 2 -5 p.m., and Saturday, June 15, from 9 a.m. to 1 p.m., Project Access is hosting another Project Reach Out, an event open to the public to help people find out if they qualify for a health insurance program. People who attend Project Reach Out will find out if they are eligible for health care through Medicaid, Florida Kidcare, Health Care District of Palm Beach County programs including Vita Health and Coordinated Care Options 1 and 2, or Project Access. The events will be held at 3540 Forest Hill Blvd., Suite 101, West Palm Beach. The events are free and people do not need to pre-register, but patients must live in Palm Beach County. Project Access and Project Reach Out are programs of Palm Beach County Medical Society Services.
Attendees must bring proof of residency in Palm Beach County including one of the following: driver’s license; picture ID with local address; utility bill with your name on it; passport; or Social Security card (optional).
Attendees must bring proof of income including one of the following: 2012 Tax Return filed; detailed bank statements (3 most recent ones); last 8 weeks of paystubs; or unemployment benefits; or detailed letter from financial supporter (signed by them with their contact information).
