Jasmine Perez is a proud Mexican American who was born and raised in South Florida. Perez has always been drawn to a career in public service. Aspiring to utilize her professional training in a capacity where she could have a positive impact, she found a home as a social worker in case management at Broward Health Imperial Point. Perez enjoys working closely with patients, coordinating appropriate post-acute care services. She also finds it rewarding to collaborate with the hospital’s multidisciplinary team to develop safe discharge plans.
“It’s important as a case manager to become familiar with community resources so you can provide the highest quality, relevant care to patients and families based on their evolving needs,” said Perez.
A licensed clinical social worker, Perez earned a master’s degree in social work and bachelor’s degree in Psychology from Florida Atlantic University.
