May 5, 2022 Cleveland Clinic will collaborate with Medically Home Group Inc. to provide acute and post-acute hospital-level care to patients in their homes, initially in the Florida region.

Through Cleveland Clinic’s Care at Home program, Florida patients will receive hospital-level care at home overseen by Cleveland Clinic medical staff. Patients will be monitored virtually through a clinically integrated, physician- led virtual command center, which will operate 24 hours a day to ensure seamless care for patients.

The program aims to deliver care over a period of time to effectively stabilize patients and allow them to maintain a stable baseline after an episode of care. Patients can receive medication management, laboratory services, IV antibiotics, infusions, physical, occupational and speech therapy, respiratory therapy, imaging, and meals and nutrition all in their homes.

“The Care at Home program is reflective of our commitment to provide innovative, high- quality care by combining technology and clinical expertise to deliver in-home, direct, hands-on patient care,” said Conor Delaney, MD, PhD, CEO and President of Cleveland Clinic’s Florida region. “We believe this unique and non-traditional care model will facilitate healing, increase patient satisfaction and improve outcomes.”

Richard Rothman, M.D., Regional Chief of Medical Operations for Cleveland Clinic Florida, said the Care At Home Program will aim to deliver the next generation of acute hospital-level care for Florida patients. Dr. Rothman, who will be the lead for this initiative in the Florida region, also said, “Patients are able to receive complex hospital-level care in a comfortable environment.  The way we deliver this care today is being transformed to improve all care delivery for tomorrow.”

Medically Home Group Inc. and Cleveland Clinic will implement clinical protocols and utilize technology with clinical and ancillary services to safely deliver care for patients in their home.

The Florida program is planned to begin in the fall of 2022.
