South Broward Community Health Services is now Memorial Primary Care, a name change that reflects the continued patient-centered care that its network of facilities have been providing for more than 20 years. 
 
 
As an integral part of Memorial Healthcare System’s commitment to providing healthcare services to patients, regardless of their ability to pay, Memorial Primary Care is dedicated to delivering high-quality, cost-effective, patient-centered primary care. With locations throughout south Broward County including Hollywood, Dania Beach, Hallandale Beach, Pembroke Pines and Miramar, a team of physicians, nurses, social workers, a nutritionist and support staff work together to meet each patient’s healthcare needs in a friendly and culturally sensitive environment. 
 
“Primary care is the backbone of population health that focuses on treating the entire person, not just individual body parts, so the care is personalized,” said Jennifer Goldman, MD, Medical Director of Memorial Primary Care. “We are proud to lead the system in making our patients, and our communities, healthier because on an individual level, everyone should have a primary care physician and team to serve as a patient-centered medical home where the focus is on wellness, so our goal is to prevent disease before it starts.”
 
The Memorial Primary Care team manages patients’ overall health and well-being, including routine care, preventative treatment, injury and illness. Each health center serves as a patient-centered medical home. Prevention of disease is the goal of the family medicine or internal medicine trained primary care physician. The role of our primary care physicians is to coordinate the overall wellbeing of each individual including referral to specialist when needed.

The National Committee for Quality Assurance (NCQA) has consecutively awarded Memorial Primary Care Centers the Patient Centered Medical Home (PCMH) Recognition Level 3—the highest-level award of its kind. This nationally acclaimed award recognizes medical practices for adopting a care-delivery model that places the patient at the center of care.

Since primary care providers focus on prevention and wellness, we reduce avoidable emergency room visits and hospitalizations.
