Orange Care Group LLC’s independent Accountable Care Organizations and Independent Physician Association have become the first to collaborate with a major health system to adopt Epic’s Healthy Planet through the Connect program. The adoption is made possible through a technology partnership between Orange and Memorial Healthcare System, which will positively impact the health of patients and private practices participating in Orange’s various value-based contracts in South Florida, New York, and New Jersey. 

 
While many Accountable Care Organizations and Clinically Integrated Networks use Epic’s Healthy Planet platform to manage patients across their disparate Electronic Health Records (EHRs) and networks, Orange Care Group and Memorial Healthcare System will be the first independent healthcare organizations in the country to share the comprehensive population health management platform to manage the care of patients both collectively and at an individual level. For patients shared between the two organizations, Healthy Planet will bring disparate health information together to establish more continuous health records. For those patients unique to Orange Care Group, the organization will be able to independently manage their own patients through a robust care management platform and predictive analytics, as well as inform the company’s internal operations, patient engagement, and provider empowerment strategies across their value-based contracts throughout the states they serve. 
 
From the same platform, Orange Care Group will also be able to offer Epic’s EHR to Orange providers, combining the strength of an integrated population health system with the industry’s top-rated EHR. The technology will allow providers within their independent ambulatory practice to identify and close gaps in patient care by utilizing the tools within Epic to stratify patients, integrate care management and coordination, and ultimately promote better outcomes. 
 
This innovative alliance between Orange and Memorial, in one of the most highly competitive and fragmented healthcare markets in the nation, will provide access to the nation’s largest interoperability framework to actively share information with more than half of the US patient population.
